Our team is passionate about serving the people of Oakland County. *NOTE: All employees are required to receive the COVID-19 Vaccine.Position DescriptionThis position will function as an essential team member for Honor Community Health care management and health home projects. The Community Health Worker (CHW) engage in collaboration with multiple team members of an interdisciplinary team to deliver integrated, person-centered, and comprehensive care to eligible beneficiaries to successfully address the complexity of comorbid physical and behavioral health conditions. This position will work directly with patients and their representatives to assist in the delivery of six core services: comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support, and referral to community and social support services. This position requires tracking and follow up, ensuring details of care are documented in the Electronic Health Record and population health management tool. Effective communication, organization, efficient time management skills, and the ability to successfully multi-task are essential to success in this position. This position will be working with in our school-based health centers and may require travel between sites.This position reports to the Nurse Care Manager. Our standard hours are Monday-Friday. ResponsibilitiesPromote self-advocacy by assisting recovering persons to have their voices fully heard; their needs, goals and objectives established as the focal point of rehabilitation and treatment. Assist with day to day operations of the health center including rooming patients, scheduling appointments, conducting outreach to patients referred to MAT, arranging transportation,Identify barriers (internal and external) to full participation in treatment and recommend strategies for improving access and overcoming barriers.
