Job Description:

Certified Case Manager to enhance our multidisciplinary team,
primarily focused on our comprehensive case management services.
This role is pivotal in facilitating our holistic approach,
ensuring patients receive continuous and efficient health care that
resonates with our high standards. The Certified Case Manager will
be responsible for assessing, planning, implementing, coordinating,
monitoring, and evaluating the options and services required to
meet an individual’s health needs. Conduct comprehensive
assessments of patient needs within a clinical or home-based
setting. Develop individualized care plans in collaboration with
healthcare professionals, ensuring the alignment of treatments with
patient goals. Monitor and evaluate patient progress and adjust
care plans as necessary. Facilitate communication between patients,
family members, and multidisciplinary healthcare teams. Manage
resources to ensure patient needs are met effectively and
efficiently. Ensure compliance with healthcare regulations and
standards. Proven ability to coordinate complex case loads. Strong
interpersonal and communication skills. Deep understanding of
healthcare procedures and regulations. Ability to work
independently and make critical decisions regarding patient care.
Medical Insurance Dental Insurance Vision Insurance Life Insurance
401k Certified Case Manager (CCM) credential required. Valid
nursing or social work license in the state of practice.
