Job Description:
Encompass Health Corporation, a leading provider of inpatient
rehabilitation and home health services, is seeking a dedicated
Certified Case Manager to join our interdisciplinary care team. In
this vital role, you will coordinate with healthcare professionals
to deliver patient-centric care plans that support optimal health
outcomes. Our ideal candidate will demonstrate expertise in
assessing patient needs, managing clinical data, and facilitating
patient transitions between care settings. Assess new and ongoing
cases, identifying patient needs and potential barriers to care.
Develop, implement, and revise individualized care plans in
collaboration with healthcare professionals. Facilitate regular
case reviews and update care plans based on patient progress and
feedback. Coordinate services between healthcare providers,
patients, and families to ensure integrated and comprehensive care.
Advise patients and families on healthcare systems, facilities, and
resources. Monitor and report on the outcomes of care plans,
suggesting adjustments and improvements as required. Ensure
compliance with healthcare regulations and standards. Strong
organizational and communication skills Ability to make critical
decisions under pressure Proficient in healthcare management
systems and databases Strong understanding of patient privacy laws
and ethics Excellent coordination and time management skills
Medical Insurance Dental Insurance Vision Insurance Certification
from the Commission for Case Manager Certification (CCMC) or
similar
