Job Description:
Encompass Health Corporation, a leader in integrated healthcare
services, is seeking a dedicated Certified Case Manager to join our
team. This role is crucial for providing comprehensive case
management services across our network of inpatient rehabilitation
hospitals, home health agencies, and hospice agencies. The ideal
candidate will coordinate and enhance patient care by assessing
treatment plans and facilitating communication among patients,
families, and healthcare providers. This role demands a high level
of empathy, patient interaction, and a deep understanding of
patient care planning in a rehabilitative environment. Assess
patient needs and create individualized patient care plans.
Coordinate multi-disciplinary healthcare services to ensure timely
and appropriate resources are available to the patient. Act as a
liaison between patients, families, and healthcare professionals.
Regularly review treatment plans with healthcare providers and
adjust plans as necessary. Maintain comprehensive documentation of
patient care and communicate patient progress to relevant parties.
Ensure compliance with healthcare regulations and standards. Strong
organizational and communication skills Ability to work
collaboratively in a multidisciplinary team Expertise in developing
care plans Proficiency in documentation and case management

Licensed Registered Nurse (RN) or Licensed Social Worker (LCSW)
