As a crucial part of our social services department, the Certified Case Manager will work across both facility-based and home-based settings to facilitate effective patient-centric case management. This role requires a strategic thinker with a solid background in healthcare management, capable of navigating complex health systems to enhance patient outcomes and optimize healthcare delivery. Develop, implement, and review individualized patient care plans Coordinate with medical professionals and support staff to provide seamless care transitions Ensure compliance with healthcare regulations and patient rights Assess patient needs and facilitate access to appropriate healthcare services Monitor and report patient progress, as well as adapt plans as necessary Educate patients and families about health management and resources Maintain patient confidentiality and document patient interactions and updates in the case management system Strong communication and interpersonal skills Excellent organizational and time management abilities Proficient in case management software and medical records analysis Critical thinking and problem-solving skills Compassionate and ethical mindset Medical Insurance Dental Insurance Vision Insurance Life Insurance Certified Case Manager (CCM) credential mandatory Licensed Registered Nurse (RN) or Licensed Social Worker (LSW), if applicable
