RESPONSIBILITIES/ESSENTIAL FUNCTIONS
The Home-Based Clinician in the Specialized Services for Youth department assumes the responsibilities common to all direct service and administrative positions as described in the Agency Manual. The Home-Based Clinician in the Specialized Services for Youth department will develop and use the various modalities of service offered by the agency.
The Home-Based Clinician in the Specialized Services for Youth department reports to the assigned Home Based Supervisor, Specialized Services for Youth (SSY) department.
The following represents a summary of the major duties and responsibilities performed by incumbents in this position. Incumbents may be requested to perform related tasks other than those specifically presented.

SPECIFIC DUTIES/JOB EXPECTATIONS
Professional Practice

  • The Home Based Clinician provides both clinical and case management services to children with Severe Emotional Disturbance who meet State Medicaid guidelines for service.
  • The Home Based Clinician provides individual, family and group therapy services as indicated in the Individual Plan of Service (IPOS).
  • The Home Based Clinician can expect to work at minimum 3 evenings a week to accommodate school and work schedules.
  • The Clinician provides appropriate home-based services to a caseload in accordance with agency needs and the skill level of the worker. Typical caseload size will be between 10-15 children and families depending on severity and may be subject to change based on program and consumer needs.
  • The Clinician is responsible to: obtain, analyze and evaluate case data; develop an understanding of the client's situation and to develop goals and planned responses; carry out the plan and/or alter the plan as new understandings demand; analyze and evaluate the course and results of the treatment process, utilize evidenced based practices as indicated. The clinician is responsible to work with the whole family; to have a good understanding of the family situation and dynamics and how this may impact the identified youth.
  • Maintain clinical records in accordance with Agency, licensing, accreditation, Oakland Community Health Network (OCHN), Michigan Department of Health and Human Services (MDHHS) standards and requirements. Maintain all records in the Electronic Medical Record (EMR) within timeframes required by OCHN.
  • Actively coordinate cases with treatment team, peer specialists, school staff, probation, courts, protective services, OCHN and any other appropriate staff.
  • Train providers of adjunct services such as peer supports, community living supports and respite services on the IPOS and document in the record.
  • Participate in professional development activities in accordance with state licensure requirements and any requirements of the funder, specifically in accordance with the Medicaid provider manual and OCHN requirements.
  • Complete mandated trainings annually and as required to maintain eligibility to provide services under MDHHS Medicaid requirements.
  • Participate in Evidenced Based Cohort Training as offered.
  • Additional duties may include: client intake; supervision of interns, assignment to specific agency work sites; assignment to a special project or contract; provision of leadership to educational, counseling or volunteer groups; to work with community problems as interest and skill indicates; and other duties as assigned.
  • Participate in the SSY crisis on-call coverage for clients by signing up for a minimum of 3-4 weeks each year.
  • Provide direct face to face client contact minimum of one and one half hour per week and provide minimum of one half hour of collateral contacts every two weeks. Monthly average of contacts must be 6 hours as face to face and 2 hours a month of collateral.
  • Attend weekly department meetings and trainings as scheduled. Attend specialized supervision groups for TF-CBT, SUD and high risk as assigned by supervisor.Administration
    • Perform the administrative tasks related to assigned caseload and other work assignments as required by the agency.
    • Perform selected and additional administrative assignments as required, including student supervision.
    • Provide assistance, support and leadership to other staff members through consulting, teaching and other means as appropriate, and to coordinate program activities when assigned to do so.QUALIFICATIONS:
      • Successful completion of the requirements of a master's degree from an accredited school of social work or equivalent in related field such as psychology, social work, or guidance and counseling as appropriate to the needs of the particular position.
      • State certification or licensure at the minimum level possible, commensurate with accrued experience and degree obtained.
      • CAFAS/PECFAS certification at the time of hire, or within three months.
      • A minimum of one year of experience in the examination, evaluation, and treatment of minors and their families.
      • Other special abilities, skills and credentials such as experience working in the Community Mental Health System, substance abuse qualifications, experience with youth with serious emotional disturbances, family systems work, trauma-informed care, delivery of home-based models and use of evidenced based models.
      • Special abilities and skills necessary to perform the required tasks and that best meet the needs of the agency also will be considered. Any standard above may be waived when compensating specifications or circumstances exist as long as they do not preclude all state Medicaid requirements to provide Home Based services.
      • Department of Health and Human Services (DHHS) and other required community meetings to keep current with all DHHS requirements as well as to network for community services for clients.
      • Handle and coordinate ongoing personnel responsibilities of the department; i.e., training records, participate in recruitment, conduct interviews and complete new hire documentation.
      • Complete reports in a timely manner as required by the Department of Health and Human Services (DHHS) policy's and COA.
      • Represent the Agency in a positive manner internally and in public.Personnel Administration
        • Interview and recommend new employees for hire, provide case management supervision to foster care workers and interns, and report to Director regarding activities of all staff.
        • Maintain a supervisory ratio of no more than five assigned foster care workers.
        • Foster appropriate interpersonal relationships, including giving and receiving feedback constructively.
        • Assign clients to staff and monitor ongoing progress of clients through regular supervision and case conferences.
        • Monitor staff compliance with personnel policy manual and Agency Policies.
        • Demonstrate ability to provide staff training; to supervise, develop and retain competent staff.QUALIFICATIONS
          • Master's Degree in Social Work or related field with appropriate licensure, with minimum of three years of experience as a social service worker in an agency; or a Bachelor's Degree in Social Work or related field with appropriate licensure and four years of experience as a social service worker.
          • The incumbent should have a minimum of 3 years of work experience in a child welfare agency or an agency performing a child welfare function is required.
          • Successful completion of Child Welfare Training Institute (CWTI) training is required.
          • Training, experience, demonstrated initiative and demonstrated competence in the field of foster care.
          • Demonstrated competence in the issues of separation and loss as it relates to the foster care population. Knowledge of the needs of children and families and the dynamics of the foster care population.
          • Knowledge and understanding of clinical issues facing families who are experiencing dysfunction. Knowledge of the needs of children and adoptive families and the dynamics of special needs adoption.
          • Leadership ability and supervisory skills.
          • Provide field instruction to student interns from affiliated colleges and universities.
          • Ability to guide and contribute to effective teamwork.
          • Flexibility in work schedule to accommodate the needs of the Agency and program.
          • Foster appropriate interpersonal relationships, including giving and receiving feedback constructively.
          • Demonstrate ability to provide staff training; to supervise, develop and retain competent staff.
          • Provide courteous interaction with co-workers and the public.
