Honor Community Health is currently recruiting for a full time
Coordinator, Referrals and Diagnostics. Reporting to the Director,
Enabling Services and Operations, Referrals and Diagnostics is
responsible for receiving, reviewing, and coordinating referrals to
other providers and diagnostics ordered for patients. The
Coordinator, Referrals and Diagnostics plays a key role in ensuring
patients are supported in accessing referral services and
diagnostics, and that these services are appropriately monitored,
followed up on, and recorded in a patients medical record.
- High school diploma or equivalent is required,
- Associates degree in a relevant field preferred.
- Completion of a technical program and possession of
certification for medical assisting preferred
- At least one year of experience working within a family
medicine or similar outpatient healthcare environment
- Strong computer skills and experience using Electronic Medical
Records as well as knowledge of Microsoft Office and internet are
- Experience working within a Federally Qualified Health Centers
or other organization supporting underserved populations is highly
